How to join
New to MO HealthNet ?
Healthy Blue is a health plan that serves MO HealthNet Managed Care members. We help coordinate and manage your health care needs. Before you can choose Healthy Blue as your health plan, you will need to apply for MO HealthNet Managed Care benefits with the Missouri Department of Social Services Family Support Division.
1. Apply for MO HealthNet Managed Care benefits
The best way to know if you are eligible is to apply. Enrollment is year-round. You can apply at any time.
Apply in one of these ways:
Call the Family Support Division (FSD)’s FSD Information Center at 855-373-4636, Monday through Friday from 6 a.m. to 6:30 p.m., Saturday from 8 a.m. to 5 p.m. or Sunday from 10 a.m. to 3 p.m.. You can also request a paper application.
Visit a local Family Support office.
2. Check your mail
The Family Support Division will send you a notice telling you if you qualify for MO HealthNet Managed Care. If you are eligible, you will receive an MO HealthNet Managed Care welcome letter and MO HealthNet ID card. You will also receive an enrollment packet to help you change your health plan if you decide to do so.
3. Change to Healthy Blue as your MO HealthNet Managed Care health plan
You can change your health plan and choose your PCP one of these ways:

Call the MO HealthNet Managed Care Enrollment Helpline at 800-348-6627 (TTY 711) Monday through Friday from 7 a.m. to 6 p.m. (except holidays).
Mail your completed change form in the prepaid envelope to:

Have MO HealthNet Managed Care and want to switch to Healthy Blue as your health plan?
You can change your health plan for any reason during the first 90 days of becoming an MO HealthNet Managed Care plan member. After that, you can change it in cases of medical necessity. To speak with someone about changing your MO HealthNet Managed Care plan, call the MO HealthNet Managed Care Enrollment Helpline at 800-348-6627 (TTY 711).
